Application of laser in Dentistry

Laser has been practically applied in oral surgery for nearly 30 years, and it has become a routine oral treatment, which can be used as an auxiliary treatment, also become an independent new project of dental equipment. Normal dental laser treatment is subordinate to the category of laser dentistry. Laser dentistry includes periodontal treatment, assisted periodontal regeneration, dental implantation, minimally invasive in oral surgery, dental restoration, endodontic treatment, Oral and maxillofacial surgery, orthodontics, etc.

 

Laser makes the diagnosis and treatment process easier and faster, reduces treatment stress of patient, and has been widely recognized in the treatment of dental and oral diseases .

 

Treatment program introduction

According to the optical properties of the oral tissue, four different ways of interacts between the light energy of laser and the target tissue including reflection, absorption, projection and scattering. In addition, for photobiological effect, it has photothermal effect, photochemical effect, fluorescence effect and micro-blasting effect.

 

For the main treatment projects of oral treatment, frequently used lasers in the world include: 450,650,808, 980,1064nm semiconductor laser, Nd:YAG laser, 2940nm(2.94um) erbium laser and CO2 laser. The following is a brief introduction of some oral treatments:

 

Periodontal treatment:

 

808, 830 and 980nm are suitable for non-surgical periodontal treatment, with bactericidal effect and are good for coagulation; Nd:YAG 1064nm’s absorption of melanin is the most, absorption of hemoglobin is less, slight absorption in water, it has bactericidal, hemostatic effect; Er:YAG at 2940nm(2.94um) and ErCr:YSGG at 2780nm are the peaks in water absorption, and with poor hemoglobin absorption. Due to the presence of water mist, the heat increase is small, which limits the hemostasis and coagulation effect, but there is no discomfort after surgery, healing speed is faster. Er:YAG 2940nm(2.94um) is more widely used in erbium laser.

 

Laser oral hemostasis:

 

Nd:YAG’s 1064nm and 808, 830, 980nm semiconductor lasers are more easily absorbed by heme. For tissues with high heme concentration, a good hemostatic environment is created after surgery. However, the hemostatic mechanism of CO2 laser is different, because the collagen in Vascular wall decomposes collagen spiral polymer after absorbing CO2 laser. This structural change leads to collagen fibers and vascular lumens contract, thus lead to a hemostatic effect.

 

For patients taking anticoagulant drugs such as aspirin and platelet aggregation inhibitors, including people with affect clotting who taking vitamin E, ginkgo biloba, cinnamon, chili, curry and other herbs and spices that are rich in salicylic acid, laser treatment does not present anticoagulant problems. Due to the poor hemostatic performance (not easily absorbed by heme) of erbium laser (2940nm/2.94um)  after gingival resection, semiconductor laser, Nd:YAG, or CO2 lasers will be used usually for blood coagulation after surgery.

 

Crown lengthening:

 

Crown lengthening can be used to repair the appearance by gum contouring to improve the patient’s smile (i.e. avoid excessive gum exposure when smiling), sometimes with a surgical guide, laser incisions are made along the edge of the guide. For 808, 830, 980nm semiconductor lasers and Nd:YAG 1064nm lasers, its target is the gingival soft tissue, can only use Er:YAG 2940nm(2.94um) lasers if bone structure needs to be changed during the crown lengthening. Semiconductor lasers are well used for anti-inflammatory, ablative, soft tissue cutting, but Er:YAG can also well used in hard tissue cutting.

 

Frenectomy:

 

All lasers with different wavelengths can be used for frenectomy, but the penetration depth of semiconductor laser and Nd:YAG (500μm) is much higher than that of erbium laser (2940nm/2.94um)) and CO2 laser (5-40 μm), and be well controlled during surgery to prevent periosteal and jaw damage.
